Learning from the tragic lessons of the Qinyuan "5.22" mine disaster, our company is comprehensively deepening its special self-inspection and rectification work on production safety.

At 19:29 on May 22, 2026, a major gas explosion occurred at the Liushenyu Coal Mine of Tongzhou Group in Qinyuan County, Changzhi City, Shanxi Province. There were 247 workers underground at the time. According to the official investigation, the accident resulted in 82 deaths, 2 missing persons, and 128 injuries. It is a serious and egregious safety accident in the mining industry in recent years, highlighting its severe nature and painful lessons. The accident exposed numerous serious violations and hidden dangers within the company, including falsifying blueprints, exceeding the authorized number of workers underground, inadequate personnel location management, a malfunctioning gas safety monitoring system, and illegal mining at concealed working faces. Safety management systems were ineffective, and the company's primary responsibilities were not fulfilled. This tragic incident serves as a stark warning to the entire industry regarding safety.
The CPC Central Committee and the State Council attached great importance to the handling of this accident, issuing important instructions and directives at the first opportunity. The State Council established a special accident investigation team to thoroughly investigate the root cause of the accident, adhering to the principle of "four no-tolerance" (no letting go of any aspect of the accident, no leniency in the face of consequences, and no leniency in the management of the accident itself), and to hold the companies involved and relevant regulatory bodies strictly accountable. Multiple departments in Shanxi Province and Changzhi City mobilized hundreds of professional mine rescue personnel overnight to the scene to carry out search and rescue, treat the injured, and handle the aftermath. This tragic news is deeply regrettable and serves as a profound warning for our company's safety management across the entire supply chain, including production and operation, raw material procurement, and mining and refractory/bauxite operations.
To deeply learn from the tragic lessons of this major safety accident, firmly establish the development philosophy of "people first, life first, safety first," and prevent similar safety hazards from occurring in our cooperative plants, storage bases, and upstream mining supply chains, the company's management urgently convened a special meeting to deploy safety production measures. Effective immediately, a comprehensive, company-wide safety production self-inspection and rectification campaign will be launched, focusing on five key areas for rectification:
1. Safety Qualification Investigation of Upstream Suppliers and Mining Partners: For our bauxite and ore raw material cooperative mines and smelting plants, we will verify their safety production licenses, underground safety management records, gas/dust monitoring equipment operation and maintenance records, and underground personnel post-work and location management systems one by one. For partners with safety management loopholes, incomplete licenses, or illegal production, supply cooperation will be suspended, and cooperation will be terminated if rectification and acceptance are not completed within a specified period.
2. Closed-Loop Inspection of Safety Hazards in Factory Production Workshops: Conduct a comprehensive overhaul of explosion-proof, ventilation, gas monitoring, and fire emergency equipment in the workshops; review job safety operating procedures; strictly investigate violations of regulations, operation of malfunctioning equipment, and perfunctory safety inspections; establish a hazard log, and implement closed-loop rectification with designated personnel, timelines, and standards.
3. Company-wide Safety Awareness Education and Skills Training: Organize all employees to study the official report on the Qinyuan "5.22" accident, analyzing the evolution logic of the accident hazards; conduct specialized training for each job position on explosion-proof and emergency escape, and hazard identification; implement pre-job safety assessments for both new and experienced employees, and strictly prohibit those who fail the assessment from working.
4. Re-examination and Implementation of Safety Management Systems: Improve the company's safety inspection, hazard reporting, and reward/punishment accountability mechanisms; implement the dual responsibility of safety production for all levels of management personnel; eliminate the "paper-based" and "formal" nature of safety systems; ensure smooth channels for employees to report safety hazards to higher levels; and implement a reward system for hazard reporting.
5. Special Control Measures for Warehousing and Logistics Transportation A comprehensive investigation was conducted throughout the entire process of loading, unloading, warehousing, and transportation of ore raw materials. Hazards such as flammable and explosive materials and excessive dust were identified. The management of hazardous chemical supplies was standardized, and the safety qualifications of cooperating transportation fleets were rigorously reviewed.
At a special meeting, company leaders emphasized that there is no room for complacency in safe production; a single violation could lead to an irreparable tragedy. All departments must learn from the Qinyuan accident, abandon the one-sided business mentality of prioritizing profits over safety, conduct hazard identification before accidents occur, and implement safety control in every detail of production. Going forward, the company will routinely conduct monthly safety spot checks and quarterly comprehensive inspections to continuously strengthen the safety defense line across the entire chain, safeguarding the lives of employees and the company's stable operation through rigorous and pragmatic management.
May the deceased rest in peace. We extend our deepest condolences to the miners who perished in this accident and their families.
